Projects for Assistance in Transition from Homelessness (PATH), is designed to provide comprehensive community based services for the homeless individuals that have a mental illness or co-occurring disorders.

Eligibility:

Homeless individuals with a mental illness or co-occurring disorders; PATH Eligibility: Must be 18 years or older with a serious mental illness or co-occurring disorder, be homeless or at imminent risk of becoming homeless; must live in: Benton, Carroll, Henry, Gibson, Lake, Obion, or Weakley Counties
